Permanent Clinical Informed Consent

INFORMED, CLEAR, AND VOLUNTARY CONSENT FOR CONDUCTING A VIDEO CONSULTATION
(IN ACCORDANCE WITH STANDARD NO. 015/2013 OF THE Directorate-General of Health)
1) Identification of the Client (information provided in the video consultation appointment request)
2) Description of the Procedure:
₋ Tele- and video-consultation with audio and video of the client within the context of the Permanent Video-Consultation Service (SVCP).
3) Objective:
₋ Advice and/or triage of situations considered a priority, which can be managed without: a physical examination, performing an immediate test, administering medication in a healthcare setting, or carrying out a medical procedure.
4) When Not to Use
₋ This service is NOT suitable in cases of:
- Urgent situations requiring a physical examination, an immediate test, administration of medication in a healthcare facility, or a medical procedure.
- Emergencies such as chest pain or tightness; weakness in an arm, drooping mouth, or difficulty speaking; altered consciousness like drowsiness or unresponsiveness; choking; heavy or uncontrolled bleeding; severe burns; or injury from an accident. In these cases, contact 112.
- Chronic disease management.
- Requests for chronic medication prescriptions, especially psychiatric medication.
- Requests for complementary diagnostic tests for evaluating non-acute health conditions or disease.
5) Advantage
₋ Remote clinical evaluation, providing comfort and ease of access.
6) Safety
₋ The electronic systems used incorporate security protocols to protect the confidentiality of your identity and clinical data, and include measures to ensure data integrity.
7) Disadvantages
- Situations that cannot be resolved via video consultation and require a subsequent in-person consultation;
- Technical difficulties or failures that prevent or interrupt the tele- or video-consultation;
- Failure of security protocols compromising the privacy of clinical data;
- Security failures attributable to the client’s device and/or network used to connect to the video consultation.
8) Duration
The duration of the consultation will be appropriate to the clinical situation within the context of a digital health service.
9) Medical Prescriptions
Based on the medical context and FHVC practices, after a consultation, if the doctor deems it appropriate according to medical criteria and in accordance with FHVC protocols, a medical prescription may be issued.
Issuance of a prescription is not guaranteed following the consultation. The decision to prescribe medication is made solely by the doctor based on their evaluation.
This service IS NOT INTENDED for the issuance of chronic medication prescriptions, particularly psychiatric medication, which should be managed by the attending physician.
